Care Management Healthfirst Navigator (P/T)

SAGE is the country’s largest and oldest organization dedicated to improving the lives of LGBTQ+ older people. Founded in 1978 and headquartered in New York City, SAGE is a national organization as well as a local direct service provider in New York City that offers supportive services and resources for LGBTQ+ older people and their caregivers, advocates for public policy changes that address the needs of LGBTQ+ older people and provides cultural competency training for aging providers.

SAGE partnered with DFTA and Healthfirst to assist members with obtaining healthcare. The goal of the Healthfirst Helping You program with Community Care Link is to better connect Healthfirst members with healthcare and social service providers so older adults can remain healthy and active in their communities. The Care Management Healthfirst Navigator (“Navigator”) aims to address Healthfirst members’ social and economic determinants of health and refer them to relevant services and supports.

Salary: This position is part-time (20 hours per week at $20 per hour) and remote. The Care Management Healthfirst Navigator reports to the Assistant Director of Care Management.

Duties/Responsibilities

Primary Care Physician (“PCP”) Engagement & Alignment Outcomes

  • Navigator will either verify the current PCP or identify a new one and ensure that an appointment with PCP has been scheduled and attended within a 6–9-month timeframe
  • Navigator will assess and address, if possible, the reasons for not visiting a PCP within the past 12 months
  • Navigator will educate the member on the role of a PCP and the importance of preventive care in general

Closing Care Gaps

  • Navigator will assist the member with accessing Healthfirst services to close gaps in members’ care that are not currently being accessed

Social and Economic Determinants of Health Outcomes

  • Navigators will assess for and make referrals to DFTA-funded services, as well as other needed services, as needed
  • Services may include Food, Transportation, Case management, In-home medical visits, financial services, Physical/occupational therapy, Senior center/day program referrals, and mental health services

Patient Perception of Health (PPH)

  • For successfully reached members, Navigator completes the PPH, which is an assessment of the program database, to better understand members’ unique needs and put them in touch with appropriate resources and services. PPH is completed every six months per member

Track outreach and activities by entering comprehensive notes into the database, “Hyphen”

Navigator will connect members to SAGE for services when applicable

Required Skills/Qualifications

  • Associate or bachelor’s degree
  • Strong customer service experience, preferably over the phone
  • Strong organizational skills, documentation, written and verbal communication skills
  • Experience working with aging populations, including LGBTQ+ older adults and BIPOC communities
  • Ability to work independently as well as be an involved member of the Care Management team
  • Self-directed and able to work with light supervision
  • Computer literacy, with knowledge of Microsoft Office and the ability to learn other pertinent software systems and databases
  • Fluency in Spanish is strongly preferred
