Founded in 1978 and headquartered in New York City, SAGE is a national organization as well as a local direct service provider in New York City that offers supportive services and resources for LGBTQ+ older people and their caregivers, advocates for public policy changes that address the needs of LGBTQ+ older people and provides cultural competency training for aging providers.
Department: Care Management
Report to: Irene Tsikitas, Director of Care Management
Schedule: Full-time (hybrid), 35 hours per week, 9:30 am-5:30 pm; hours may fluctuate based on client home visits.
Salary Range: $70k-$75k annually
Position Overview: The primary function of the Assistant Director – Caregiving is to assist with the administration of SAGE’s Caregiving Program. The Assistant Director – Caregiving will work closely with the Director of Care Management and Compliance/Administrative Assistant to ensure that reporting, systems, and procedures are done correctly and consistently across the Caregiving program and will help coordinate preparation work for DFTA site visits.
Duties/Responsibilities
- Oversee the Caregiving program and supervise Caregiving Care Managers
- Conduct supervisory reviews on all new client cases within the DFTA grant-mandated timeframe (10 days) and follow up with Care Managers when errors arise in case notes and service delivery; a monthly review of existing cases.
- Complete all administrative paperwork required by DFTA and assist in reporting process as required
- Provide caregiving support services/groups, case management, and information and referrals to identified clients and caregivers throughout New York City
- Carry a caseload and perform client assessments as well as home visits as needed
- Conduct client intakes and refer eligible participants to other SAGE services as needed/relevant
- Design, launch, and support groups as needs are identified
- Assist clients and their care recipients with public benefits applications and other entitlement programs and opportunities
- Conduct workshops, presentations, and training related to caregiving
- Conduct advocacy for clients on an as-needed basis with other service entities
- Enter client records & notes, and collect all participant and service unit data as outlined by organizational policies
- May be assigned other tasks and duties reasonably related to Job responsibilities
Required Skills/Qualifications
- LMSW required or comparable degree
- Supervisory experience
- Experience performing case management
- Knowledge and/or experience working in LGBTQ+ communities, specifically with older adults
- Computer skills (Excel, PowerPoint, Publisher, etc.) and experience working in client databases, preferably STARS, and Salesforce
- Networking & presentation skills
- Fluency in Spanish preferred
Special Requirements
- Local travel required to clients’ homes and to other organizations & meeting places
- Some irregular hours, with some evening and weekend time required with advance notice
